Hamilton Health Care System is now providing patients access to MyChart®, an advanced digital platform that allows individual users to view test results, request prescription refills, track health changes, manage appointments, streamline communication across providers and more.

MyChart® allows users to access information from multiple providers – both inside and outside HHCS – through a single, integrated and easy-to-use platform.
To sign up for a free account, users can visit mychart.hamiltonhealth.com, enter personal information and start exploring. Patients also have the ability to link portals they have signed into with other providers, and MyChart® automatically transfers the information.
MyChart® provides a wide range of features including:
Connecting and sharing
With MyChart®, users can access information for individual family members from a single account, view or share specific visit summaries, request medical records, see which other family members and care providers have accessed their medical records and more.
The Share Everywhere feature gives any provider with an internet connection temporary one-time access to their medical record, and Care Everywhere authorization allows providers to share medical records with other health care organizations when needed.
Happy Together provides a single view into information from health organizations where the patient has received care – including allergies, medications, health issues, visits, test results and care team members.
Health
MyChart® can notify users of test results, provide access to healthcare lists and instructions (for medications, allergies, health issues, etc.), send reminders of when preventative care like physical exams or immunizations are needed, and more.
An additional health tracking feature allows users to link personal devices that track steps, blood pressure and more to the platform, while a symptom checker can give patients recommendations for care. Patients who have a living will or advance directive can upload those documents on the platform.
Messaging
Communication between patients and providers is essential, but missed phone calls and busy schedules can complicate the process. MyChart® allows patients to send and receive messages from their providers.
Scheduling and appointments
With MyChart®, users can request and cancel appointments, get on waitlists for providers who offer that feature and complete the check-in process from home to save time at the clinic. Users can also update personal, medical and insurance information and electronically sign documents.
Hamilton telehealth providers can connect with patients through MyChart® features that allow patients to submit photos, videos and more. And when patients need care urgently, MyChart® allows them to give local emergency departments and emergency care centers notice they’re coming in. Users can even turn on the Hello Patient feature, which uses their phone’s location tracking to automatically check them in when they arrive at the clinic.
Payment and Insurance
Patients can view and pay bills on MyChart®, set up payment plans, get estimates for the cost of common procedures, request financial assistance and more.
Preferences
MyChart® leverages modern cybersecurity practices, including two-factor authentication and secure username and password recovery, to keep information safe. MyChart® is also available in Spanish.
Mobile Features
No need to wait for computer access. With the MyChart® Mobile app, users can access all of MyChart’s® features on their smartphone, and secure their accounts using fingerprint technology, face recognition, or a four-digit passcode. The MyChart Mobile app can also pair with smartwatches to receive notifications, view upcoming appointments, read messages and more.
And for patients who are hospitalized, MyChart® allows them to track their daily schedule, learn more about members of their care team, access educational material and more.
