Hamilton Emergency Medical Service operates a billing and collections center staffed with experienced ambulance coding and collection specialists that will file claims to primary and secondary insurance carriers on behalf of our customers.
We are committed to ethical business practices and compliance with applicable laws, rules, regulations and policies governing the provision of healthcare services and reimbursement.
A Billing Specialist will work with you to verify we have the appropriate signatures, forms and required documentation on file to make a submission to your insurance carrier.

Request for Legal Documentation
All patient care documents and incident reports created by HEMS personnel during the performance of their duties are protected, private information and cannot and will not be released without proper identification and legal authorization by the patient or by orders of the court in the form of a search warrant or subpoena.
HEMS has a form: Authorization to Release Information that must be completed and signed by the patient and witnessed by a HEMS employee at our office. The authorization directs any and all Federal and State agencies, Workers Compensation Section, Police Departments and other law enforcement agencies, governmental agencies, employer, corporation(s) or other entity to release and to furnish “Named Person or Company” all information and records, police reports and all related documentation and memoranda, and any other type of related information with reference to the individual named herein. A copy of this authorization shall be as good as an original and it shall never expire until revoked in writing.
If obtaining records by Search Warrant or Subpoena in lieu of a patient’s signed authorization, a copy of the legal document must be presented and attached to the signed request. All requests by warrant or subpoena will be handled by the HEMS Compliance Manager and a processing fee of $25 must be paid at the time of pickup.
All patient records must be picked up at 1105 Memorial Drive, Dalton, Georgia 30720 in person. To accelerate the process, please contact the HEMS Compliance Office at 706-278-9211 and request the documents to be retrieved a minimum of 24 hours in advance. When you arrive to pick up the documents, please bring a copy of a completed Authorization to Release Information Form, Photo Identification or proof of Power of Attorney or Executor of the Estate Forms. A processing fee of $10 will be charged to patients obtaining their medical records and must be paid by cash or check at the time of pickup.
