Dalton, GA - Hamilton Home Health has partnered with HomMed to provide in-home telemonitoring. Daily monitoring allows clinicians to detect actual or potential problems, eliminating unnecessary hospitalizations and ER visits.

"These monitor's are a tool we use to provide physicians with more comprehensive information regarding their patients and assist them in managing the care of complex disease states," said Sandra Messersmith, Director of Hamilton Home Health.

Individuals struggling with a chronic disease or other health conditions will benefit from this new technology. The HomMed Monitoring System has two components: the actual monitoring unit that is placed in the patient's home and the HomMed Central Station. The home unit is a vital-sign monitoring and communication device that measures weight, blood pressure, heart rate, blood oxygen saturation, and temperature. For Individuals with diabetes, the unit can collect glucose levels. Other capabilities include lung function, ECG readings and blood clotting times.

A patients vital signs are automatically sent to the HomMed Central Station and the data is received and reviewed by Hamilton Home Health. "If a patient's sign's fall out of their pre-set ranges, or if the patient fails to take their vital signs at the right time, we would be prompted to contact the patient immediately," reported Messersmith.

The HomMed Health Monitoring System provides patients with the security and peace of mind that comes with knowing that their health is being monitored seven days a week by their health care professional. Hamilton Home Health, a service of Hamilton Medical Center, provides this service in the six northwest Georgia counties it serves.
