The DAISY Award recognizes exemplary nursing excellence, and is the highest honor a nurse can receive at Hamilton Medical Center (HMC).

Who can nominate?

Hamilton Medical Center’s patients, visitors, nurses, physicians, volunteers, and associates may nominate a deserving nurse by filling out this ONLINE FORM.

What are the nomination criteria?

This nurse’s clinical skill and especially her/his compassionate care exemplify the kind of nurse that our patients, their families, and our staff recognize as an outstanding role model. She/he consistently meets all of the following criteria:
Compassionate – Caring Attitude – Demonstrates Excellence – Cheerful Disposition – Promotes Health – Team Player
