If you need assistance coping with any of life's problems, the Employee Assistance Program (EAP) is here to help you. The EAP is a confidential assessment and referral service. It provides professional counseling to the employees of participating companies and institutions.

EAP OVERVIEW
What is an EAP?
An EAP is a program designed to provide prevention, early intervention and diagnosis at the earliest and most cost-effective setting.
What services does an EAP provide?
The EAP provides confidential, professional counseling for employees and their families with problems affecting their personal lives or job performance.
What is the scope of services?
The EAP provides for an assessment, short-term problem resolution, referral to an appropriate behavioral health provider, follow up on efficacy of treatment, and reintegration back to work.

Who may use the EAP?
The EAP recognizes the problems at home affect the employee in that setting as well as at work; therefore, the employee and any member of his or her immediate family are eligible to use the program.
How much does it cost?
Services provided within the scope of the EAP are at no cost to the client. If services are required beyond the scope of those routinely provided beyond the EAP then the client would be referred to the appropriate clinician authorized by the health plan.
Who provides EAP services?
Services are provided by trained certified EAP personnel. If the client is found to require services beyond the scope of the EAP, the EAP counselor is obligated by professional standards (EAPA and EASNA) to refer that patient to the appropriate network behavioral provider. The EAP counselor will not assume the role as the behavioral health professional and establish a clinical relationship beyond the scope of the EAP.

EMPLOYEE ASSISTANCE SERVICES FOR
HAMILTON ASSOCIATES
Hamilton Health Care System, through the Westcott Center, provides a comprehensive Employee Assistance Program (EAP) for their associates, helping them find help for problems such as alcohol or other drug dependency, stress, marital problems, family difficulties, financial problems, grief, depression, coping with change, and problems that may adversely affect life, health and work.
In coming to the EAP (either by myself or referred by my supervisor) I understand that I have six (6) free sessions per year and that I must be a full or part-time employee (employees working at least 20 hours per week who have at least 3 months of completed service) to be eligible. I also know that my immediate family members (spouse or minor dependent children) are also eligible. I understand that if I don't keep my appointment or give 24 hour notice, that this will count as one of my free visits. I understand that my six visits must be used before December 31. Any unused visits will not carry over tot he next year.
I understand it is my financial responsibility for further visits beyond my sixth free session and I will be encouraged to access my insurance. I agree that I will be responsible for all costs not covered by my employer and/or insurance company. I will be notified when I have one free visit remaining.
If an outside Ph.D., LCSW, or physician referral is needed, the EAP will provide me with a list of Hamilton approved Psychiatrists, Psychologists, or Psychotherapists. My free visits will not cover these outside providers. From that list, I will be given recommendations as to who to use. I will be financially responsible for referrals to outside professionals and encouraged to access my insurance.
If it is determined by the EAP that I would benefit from a referral made to a Westcott Center Ph.D. Psychologist or a Westcott Center Child and Adolescent Psychiatrist, approximately sixty eight dollars ($68.25) may be applied towards the psychologist's or Child and Adolescent Psychiatrist's charges for the first six (6) visits. My insurance company can be billed for the remaining amount.
I also understand that all disclosure of information is governed by Federal and State confidentiality rules and regulations and that whatever I say in my psychotherapy sessions will be held in strictest confidence and will not be shared with anyone unless I give written consent to do so.
