Community Health Needs Assessment Survey


 

How Does It Work

The CHNA (Community Health Needs Assessment) is completely anonymous.   

Why Participate?

Hamilton is conducting a study to identify the most important health issues in our region.  The report will help us make decisions about the future of health care and disease prevention in North Georgia.   

Who Should Participate? 

We encourage all individuals 18 years or older, living or working in Whitfield and/or Murray counties to take the on-line survey.  There are only 8 questions and it should take between 5 and 10 minutes to complete.

In Appreciation

Once the survey is completed, you will have the option of entering to win an iPod touch (4th generation - 8 GB).  This drawing will take you to a separate on-line location and will not be linked to the survey in any way.

Eligibility Rules for iPod touch drawing

  • Only one entry per person
  • Must be 18 years old or older to win
  • Winner must be willing to pick up the prize at Hamilton Medical Center's primary location (1200 Memorial Drive, Dalton, Georgia) within 4 weeks of notification 

Results of Study

Hamilton Medical Center will post the complete study and results later this year on this website.
